Online SWAT Youth Group
This form is for parents/guardians of children between 14 and 17 years old who are willing to volunteer in an online Support Wall Action Team (S.W.A.T.).
Support Wall Action Teams:
Group members meet on a weekly basis on the community Discord server. Youth Groups are reserved to members under 17 yrs old and are facilitated by trained volunteers with a checked background.
Meetings include the use of text/voice chat with every other member of the Youth SWAT Group that are present.
Interactions take place with other Youth members, their Group Leader and Heartsupport Staff members in charge of the program.
I (parent/guardian) acknowledge and agree that activities performed by the Minor as a volunteer will be performed strictly on a voluntary basis, without any pay, compensation, or benefits. I agree and understand that the Minor must comply with the rules and regulations established from time to time by the Heartsupport community and that failure to do so may result in the Minor’s immediate removal as a volunteer. I agree that all volunteer activities are to be performed by the Minor at the Minor’s risk and I assume full responsibility therefore.
By signing below, the parent or legal guardian gives their consent for (Minor) to participate in the above mentioned online facilities and activities. By signing below, the parent or legal guardian also understands that Heartsupport Discord server is an open community where anyone of any age can join from anywhere in the world at any time. Although Heartsupport does its best to make the online community a safe place for all people, I (parent/guardian), understand that Heartsupport is not liable for all conversations or actions that take place within the community or in the online world.
